Ink Fingerprinting FD-258

Your fingerprints are unique. No two are the same, not even on the same person or on identical twins. Not only do your fingerprints help to identify you, but the patterns made of tiny ridges in your skin that help you to hold on to things.

Conventional Fingerprint Card (FD-258)

FBI Applicant card (FD-258) $40.00 for first card, $20.00 for second and $15.00 for each additional card. Checks for FDLE, FBI and all State and Federal Agency's in Florida.(Background check is not included)

What You Need to Bring:

1. Government Issued ID (driver license, state issued identification card, passport, military ID card and work visa with photo)
   
2. Clients must present a valid ORI#, Organization's ORI or OCA Code Number
   

ORI Code: Is the Originating Agency Identifier Number which tells the Florida Department of Law

Enforcement (FDLE) the purpose of the background check.

OCA: Is the Controlling Agency Identifier provided by DCF to identify the provider requesting the

background check.

METHOD OF PAYMENT: We do not issue refunds under any circumstances once the Ink fingerprints are done (Please ensure that the information you provide is accurate).

Notary Public/Loan Signing

Serenity Notary Services: We notarize Clerk of the Court documents, Real Estate documents, Wills, Deeds, Power of Attorney documents, Affidavits for Moral Character, Medical forms, Travel Consent forms and Acknowledgements, and more... Fees are $10.00 for the first signature and stamp and $5.00 for each additional signature and stamp.
